IMPORTANT: Stool specimen must be collected and shipped on the same day.

PART 1 | SPECIMEN COLLECTION INSTRUCTIONS
1.
Record the date of collection on the labels affixed to the following:
Green-top specimen tube
White-top specimen tube
2.
Collect your stool specimen using the Raku-Ryu cup provided in your kit.
Saturate the tip of the swab into the collected stool specimen.
Break the swab off at the indicated line and place it into the collection tube.
White and green-top tubes: transfer stool sample into the tubes using the built-in scoop (4 scoops minimum; not to exceed the add specimen line).
DO NOT DRAIN LIQUID FROM CONTAINERS

Place all three tubes inside the provided biohazard bag.
PART 2 | SHIPPING INSTRUCTIONS
1.
Confirm that the date of collection is recorded on each of the labels and that the requisition form (completed by your healthcare provider) is included in the box.
2.
Place the biohazard bag with tubes inside of the insulated silver envelope, along with the cold pack, which needs to be activated by squeezing. (Detailed instructions included on the back of the cold pack.)
3.
Place the silver envelope into the StoolDx box with the patient requisition form and close the box.
4.
Place the StoolDx box inside of the pre-labeled shipping envelope and seal shut.
5.
Call our customer service department to schedule the specimen to be picked up from your residence.
